Patient Forms
Once you have made your initial appointment, we recommend that you download the patient forms below, print them, complete them prior to your arrival and bring them with you to your visit. This will help speed up the new patient registration process and get you in to see the doctor faster. Be sure to bring in your insurance card, necessary referrals and any test results that may have been ordered.

Emergencies
In the event of a life-threatening medical emergency, call medical emergency services at 911. DO NOT WAIT! Inform the emergency room staff that you are a patient of Dr. Peikari, and they will contact us.
After Hours Emergencies
If you have an urgent need outside of our regular office hours, please call (972) 964-7800. A doctor is on-call 24 hours a day.
