Once you have made your initial appointment, please download the patient forms below, print them, complete them and return them to the office prior to your visit. This will help speed up the new patient registration process and get you in to see the doctor faster. Be sure to bring in your insurance card, necessary referrals and any test results that may have been ordered.

Thursday: Dr. Peikari does facility visits; the office is open for calls.
Friday: 8 AM - 12 PM, 1 PM - 5 PM

In the event of a life-threatening medical emergency, call medical emergency services at 911. DO NOT WAIT! Inform the emergency room staff that you are a patient of Dr. Peikari, and they will contact us.
